Chronic Disease Management plans and Team Care Arrangements

We are happy to see any patients under a CDM plan or TCA. For referrals, please send all appropriate paperwork including:

​

• Signed Referral form of individual allied health services under Medicare; dated and with number of allocated services.

• Signed referral letter, including patient handover 

• Any further imaging or investigations  (supplementary)

• GP management plan (supplementary)

 

Fee: Medicare currently provide patients a subsidy of $58.30 from usual physiotherapy fees, therefore a gap payment will be outstanding. Patients are required to pay full fee at time of consult. Subsidies from Medicare are then electronically claimed for the patient, at the time of our consultation. The gap (Out of pocket) payment is therefore $41.70 on a standard appointment and $71.70 for an initial appointment, is required by the patient.

Workers compensation and Motor Vehicle accident claims

​

We are happy to accept referrals for patients with workers compensation and motor vehicle claims (ICWA). For accepted referrals, patients will need to provide:

​

• Doctors referral for physiotherapy services under a workers compensation claim

• Document of accepted Insurance liability and claim number

 

All services will be billed directly to Insurer with no patient out of pocket expense.

Please contact us regarding NDIS referrals which we may be able to assist with, on a case by case basis, depending on specific patient needs, level of care and facilities required.

Private referrals

We welcome any private referrals for musculoskeletal complaints.
